Who To Advise When You Change Your Name

When you change your name in the United Kingdom, there are numerous organisations and companies that need to be advised. Although government departments usually need to see an original document (your marriage certificate or Deed Poll), most companies and organisations will accept a photocopy, except for your bank and building society, who will need to see the original

We have listed below many of the companies and organisations you need to advise of your change of name.


Employer.
Inland Revenue (obtain your reference and address from your employer).
Department of Health & Social Security (write to the Contributions Agency at your local Social Security Office).
Doctor.
Dentist.
DVLC.
Passport Office
Bank.
Building Society (mortgage and/or savings accounts).
Credit Card and Store Charge-Card Companies.
Finance/Loan Companies.
Premium Bond Office.
Investment Companies.
Companies that you have Shares in.
Pension Company.
Insurance Companies (e.g. motor, medical, life, property, property contents etc).
Mail-Order Catalogue Companies.
Motoring Organisations.
Professional Institutes and Bodies.
Clubs, Societies and Associations.
Internet Service Provider (if your e-mail address incorporates your old name, you may wish to change your e-mail address).

I wish to advise you that following my [marriage on {date}] [change of name by Deed Poll on {date}], my name has changed from {previous name} to {new name}. Please amend your records accordingly.

I have enclosed a copy of my [marriage certificate] [Deed Poll]. Please let me know if you would like to see the original document or require any further information.

Yours faithfully,

Please note, it is not possible to change your name on your birth, marriage, adoption or decree absolute certificates.
